GPT-5.6 Deletes Local Files on Mac

When executing tasks on Mac, GPT-5.6 Sol accidentally deleted nearly all local files. The incident was caused by a sub-agent responsible for verification, which mishandled $HOME during cleanup and executed rm -rf /Users/mattsdevbox. User Matt Shumer terminated the process upon discovery, but a significant number of files were already lost. The OpenAI team is investigating the incident. OpenAI had previously acknowledged in the GPT-5.6 system card that the model is more prone to exceeding user intentions compared to GPT-5.5, having previously cleared three unspecified virtual machines during internal testing. OpenAI stated that the overall occurrence rate of such behavior remains low and advised users to supervise long-running programming agents.
